Overview
Under the direct supervision of the Director, Obesity Clinic, the Program Coordinator will be responsible for the administration and coordination of all Obesity Project program activities. Responsibilities include monitoring the screening process, enrolling clients, updating and managing client files throughout all program phases, clinic staff training for Project procedures, oversee scheduling of client sessions, tracking and monitoring client progress during all program phases, ensuring that data collection and reporting processes are accurate and valid, and tracking the referral of clients to necessary support services in the community as required.

ResponsibilitiesAdministration and coordination of all Project program activities as well as monitoring the Project enrollment and screening process.Oversee scheduling of client sessions in collaboration with the Project protocols.Tracking and monitoring client progress during all project phases.Ensuring that data collection and reporting processes are accurate and valid.Participate in development of referral tracking system and community events for outreaching activities and assist any needs clinic counseling activities for the patients eligible for the Ensure that the contractual goals and objectives of the program are met, including clinical visits, outreach numbers, and referrals.Develop, maintain and expand collaborations with appropriate community agencies.In conjunction with the Obesity Project, facilitate the evaluation process and develop an outreach plan to include Enrollment Development Specialists and Clinic Women's Health CoordinatorsParticipate in evaluating program outreach and educational materials.Perform other related duties as assigned.QualificationsHigh School diploma required. Associates Degree in Social Work, Community Programming, Health Administration, Business or other health-related field preferred.A minimum of two (2) years experience as a Maternal Health Counselor, CPHW, FPHW or Outreach Coordinator required.Bilingual preferred:
English/Spanish/Mandarin depending on location.Previous experience working with obese individuals and working with low income, at-risk populations preferred.
